Powers of attorney and guardianship forms

You can use these forms to make, change or 'revoke' (cancel) powers of attorney and guardianship.

How to use these forms

  1. Print the form/s you need.
  2. Check witnessing requirements and arrange for witnessing of the signatures. Make sure the witnesses have read the instructions set out in each document.
  3. Complete the forms according to the instructions.
  4. Photocopy and certify copies.
  5. Give a certified copy to your attorney, agent or guardian. You should also give a copy to anyone else who may need to be involved, such as your doctor, lawyer, accountant or stockbroker.
  6. Keep the original in a safe place. Every household should have a box or folder where important documents, such as powers of attorney and Wills are kept. Family or close friends need to know where these are kept. You could also carry a card in your purse or wallet that has the details of your powers of attorney.
